Print and Fill Out the following forms to fill them out:

• Student Physical Form - Filled out by a doctor

• Medical Questionnaire - Filled out by parent/guardian

Additional Forms where applicable

• Bus Transportation Form - If your child needs to be bused in

• Transportation to Childcare Form - If your child needs to be bused to a Childcare Facility

• Release of Student Records Form - If your child was in a public Pre-K outside of Rockaway Township

Required forms for students whose parents/guardians are not on home/rent documents

• Resident Affidavit - To be filled out by both the lease holder and the parent/guardian

• McKinney-Vento Housing Questionnaire - Filled out Housing Services

4. Parent Photo ID - Valid NJ Drivers License or Valid US Passport

5. Proof of Custody (if applicable) - (Divorce Decree / Notarized Statement from both parents)

6. IEP or 504 Plan (if applicable)
